A hospital colonoscopy in Austin can run $4,500 or more. Walk three miles to an ambulatory surgery center and the same procedure — same gastroenterologist, same sedation — costs $1,200. That price gap isn't about quality. It's about facility fees.

This is one of the clearest examples of why cash-pay pricing exists. And it's exactly why we built MarketCare.

What's Included in the Price?

A colonoscopy bill has three parts:

Surgeon/Gastroenterologist Fee: $400–$1,500. This is the doctor performing the procedure.

Facility Fee: $300–$1,200. This is where the surgery center makes its money. Hospital facility fees are 2–3x higher than ASC facility fees for the identical room and equipment.

Anesthesia Fee: $150–$600. Nearly all colonoscopies use propofol ("twilight sedation"). Some ASCs bundle this into the total price. Others bill it separately — always ask.

The hidden cost: Pathology. If polyps are found and removed, they're sent to a lab. That's $150–$400 extra. Not every ASC includes this in their quoted price. Ask upfront.

Cash-Pay vs. Insurance vs. Medicaid

Cash-Pay (Self-Pay) - Range: $1,200–$4,000 - Pay upfront. No surprise bills months later. - Most ASCs offer all-inclusive packages. - No referral needed at most facilities.

Insurance - Screening colonoscopies are "free" under ACA — but only at age 45+ and only if no polyps are found. If the doctor removes a polyp, the procedure reclassifies as "diagnostic" and your deductible kicks in. Many patients get surprise bills for $1,000–$3,000 after a "free" screening.

Medicaid - Range: $322–$580 (Medicaid reimbursement) - Long wait times. Limited provider availability in Austin.

When Do You Need a Colonoscopy?

The American Cancer Society recommends screening starting at age 45 for average-risk adults. Earlier if you have family history of colon cancer, inflammatory bowel disease, or unexplained GI symptoms.

You should also consider screening sooner if you experience persistent changes in bowel habits, unexplained abdominal pain, blood in stool, or unexplained weight loss.
